Bereaved Families of Ontario – Midwestern Region promises to protect your personal information and use it for its intended propose only. When becoming a BFO-MR employee, board member, volunteer or member, you choose to share personal information with our organization.
What is Personal Information? “Personal Information” is any information used to identify an individual. This information does not include the name, title, business address, business telephone number.
What information is protected? BFO-MR will protect any of your personal information which is collected.
Collecting and using your personal information: BFO-MR collects and uses your personal information for the following identified purposes:
- To verify your identity
- To register you for programs, services or events
- To comply with tax requirements
- For distribution of our newsletter and email communications
- To request donations
Our commitment when you share your personal information: When you provide your personal information to Bereaved Families of Ontario – Midwestern Region, you do so with the understanding that your personal information may be used or disclosed for the Identified Purposes.
Your consent: Your consent is required before BFO-MR may collect, use or disclose any personal information, except in special circumstances, such as police and/or legal investigations or other situations required by law.
How do you provide us with your consent? The following constitutes as your consent for BFO-MR existing use and future collection of your information. Program and event registration, memorial requests, donations, newsletter/email subscription, volunteer and library usage is where information is collected.
Consent may be implied: If you have completed any application for you and your family or anyone else you represent, it is implied that you have obtained consent from them, even though they may not be present during the application process, for the collection, use and disclosure of their personal information for the Identified Purposes.
Selling of Information: At no time will BFO-MR sell, lend or give your personal information to any group or person without your consent unless required by law. This includes list brokers, mail order businesses, telemarketers or other marketing companies to sell their services or products.
How you can withdraw your consent: You may withdraw your consent at any time upon reasonable notice, subject to legal or contractual restrictions. To withdraw your consent you must contact the office at the address, telephone number, fax number or email listed below. Alternatively, you may withdraw your consent to receive email communications from BFO-MR by unsubscribing from our distribution list using the SafeUnsubscribe link in our emails.
How long does BFO-MR keep personal information? Your personal information is kept for only as long as BFO-MR needs it to effectively provide services. BFO-MR shall use care and caution when storing and or destroying your personal information to ensure there is no unauthorized access.
How do we protect your personal information? BFO-MR ensures security systems are in place to protect your personal information be it electronic or in a paper-based format. Staff computers and file storage are password-protected and any files containing personal information are stored in locked filing cabinets. BFO-MR contractually requires any third party person or organization providing products or services on behalf of BFO-MR to protect your personal information and use it for its Identified Purpose.
Your right to access your personal information: You have a right to access your personal information that BFO-MR has in its possession. BFO-MR has the right to refuse your access to personal information if your information is protected by law.
How do you access your personal information?
Please make your request in writing to the office, stating specifically what information you are seeking.
BFO-MR will respond to any requests as soon as possible.
Keeping your information accurate. BFO-MR strives to ensure that the personal information we have collected is accurate and up-to-date. We make every effort to continuously update our records and we may contact you to update or verify current contact information.
